Important Information Regarding Your Wellness Requirements

How to Qualify for the Premier Plan of Benefits


Effective January 1, 2013, Chairman Ameris and the Trustees wanted to simplify the process and avoid future confusion pertaining to qualification for the Premier Plan of benefits. The Fund will require you and your spouse (if applicable) to complete a Health Risk Assessment ("HRA") and have a routine physical exam with the appropriate diagnostic health screenings EVERY calendar year.

If you are eligible, or become eligible, for Welfare Fund benefits in the period from February 1st through July 31st, you and your spouse(if applicable) must each complete the HRA(s) and have annual routine physical exams(s) with the diagnostic health screenings by September 30th of that year in order to qualify for the Premier Plan for the following year. Should you and your spouse(if applicable) not complete the wellness requirements you will be enrolled in the Basic Plan effective on January 1st of the following year.

If you become eligible for Welfare Fund benefits between August 1st and January 31st of any year, you will be enrolled in the Premier Plan and you and your spouse (if applicable) must complete the HRA(s) and have annual routine physical exam(s) with the appropriate diagnostic health screenings by September 30th of the following year to satisfy the wellness requirements. Should you and your spouse (if applicable) not complete the wellness requirements you will be enrolled in the Basic Plan effective January 1st of the following year.

Once you are moved into the Basic Plan, you can re-enroll in the Premier Plan by completing the wellness requirements. Once the wellness requirements have been met, you and your eligible dependents will be moved into the Premier Plan the first day of the following month.

Please note: If you are making COBRA payments for medical coverage when you are scheduled to be moved into the Basic Plan you will remain in the Premier Plan during your period of COBRA coverage only. Once you achieve eligibility through hours worked, you will be moved to the Basic Plan until the wellness requirements are met.

How do you complete your Wellness Profile? You can either fill out a paper copy that you can get from the Combined Funds office, or fill one out online. Remember, a Wellness Profile must be filled out by both you and your spouse, if applicable.

Drug Testing

In conjunction with Mobile Medical Corporation, all Local #373 members must be tested annually. When it's time to renew your Drug Free card, you will be notified by Mobile Medical Corporation and receive a packet in the the mail with detailed instructions on where and by when to report for testing.

With each annual test, you must surrender your current Drug Free card. However, you will be given a chain of custody form that will serve as a temporary card until a new one is issued in about seven days. The new card will be valid for one year from the date of testing.

What Happens When a Member Tests Positive for Substance Abuse?

  • The member is encouraged to seek treatment for Substance Abuse.

  • To initiate the process, member calls the Highmark Behavioral Health Unit ("Unit") at 1-800-258-9808.

  • The Unit will assist the member in locating the best treatment facility based on the member's preference and location for an evaluation. Most treatment facilities offer treatment evaluations at no cost to the member. Treatment evaluations may be scheduled in advance or provided on a walk-in basis.

  • During the call to the Unit, the member needs to "ask for a referral to a Highmark Behavioral Health Coach"("Coach"). The referral to the Coach will result in a follow up call from the Coach to the member.

  • When the member's evaluation by the treatment facility is completed, the facility will recommend a treatment plan for the member.

  • Upon admission to the treatment program the member will complete an Authorization to Disclose Health Information(ADHI) form.

  • Upon completion of treatment, the treatment program will inform the Coach of the member's status, i.e., treatment was completed successfully or not completed successfully.

  • The Coach will inform the Laborer's about the member's treatment completion status.

  • The Coach will remain available to the member for any additional case management or other services that may be of benefit to the member.

LOTS Program

The "LOTS" Safety Awards Program may be sponsored for a specific project at the request of a Local Union Business Manager or signatory employer representative. The program is only available to Laborers in good standing employed on that project. Awards may be earned over the duration of that project only. Exception: With prior approval, Laborers who are transferred to other projects by the same employer, may carry their earned work hours with them.

Awards pictured can be earned by working the hours listed for each hit without a lost-time accident and maintaining an outstanding record of on-time attendance.

Work hours cashed in for awards do not accumulate towards acquiring other awards. For example, if you have 1300 hours and claim a safety kit(1200 hours) from awards, you now have 100 hours left which will be added to any future hours earned on the project. You may claim as many awards for which you can earn hours, including duplicate awards, during the project sponsored. When you experience a lost-time accident and/or lapse in on-time attendance, all work hours accumulated up to that point are erased.

Lots award brochure You should keep track of your own work hours and notify your Local Union when you believe you are eligible for an award. Local Union Business Managers will verify all award requests with your employer.

Awards will be presented to winners at Local Union membership meetings, or on the jobsite. Pictures of the presentation may be taken. Please allow 2 to 3 weeks for delivery of awards.

Tony was employed by P.J. Dick at 3 PNC Plaza for several months. He was presented with a $5,000 check by site safety manager Jennifer Grove. Keep up the good work, Tony!

Unemployment Benefits

If you need to file for unemployment benefits, there are two ways to do it. One way is to file by phone. The phone system has been upgraded throughout to provide more efficient handling of claims. Changes to Unemployment Compensation in Pennsylvania (Effective 2013)

On Tuesday, January 1, 2013, Congress passed legislation that includes a provision allowing for the continuation of Federal emergency unemployment compensation benefits through 2013. This is good news for the long-term unemployed in Pennsylvania. However, also taking effect on January 1, 2013 are some changes legislated pursuant to Pennsylvania's Act 60. These changes primarily impact a claimant's financial eligibility to collect benefits, and they only impact those claimants with an application for benefits date on or after January 6, 2013. Below is a brief overview of the major changes, showing a comparison between the old law and the new law, and brief explanation of the significance of the change.
